On ground in forest of Quercus, Picea, Salix and Betula at 3550 m.Pileus 5.5 cm in diam., convex to applanate, grayish to pale gray, slightly viscid; margin striate (R=6/27). Lamellae free, white. Stipe 9.0 x 0.8-1.2 cm, white, fibrillose. Volva 4 x 2 cm, white, with yellowish to brownish spots.
